When to use the amendment form?

Use the amendment form if you are requesting additional data for an existing project.
If you are requesting data for a new project that is very similar to a previous project, and uses partly or entirely the same data, please do NOT use the amendment form but submit a new proposal. If you wish to use the exact same dataset created for a previous project, you can specify this on the form.
